RN Rehab Admission Clinical Liaison Days

AdventHealth Winter Park is hiring a full-time RN Rehab Admission Clinical Liaison for its inpatient rehabilitation unit in Winter Park, Florida, with a primary working location at AdventHealth Orlando on a Monday–Friday day shift plus a weekend rotation every 6–9 weeks at AdventHealth Winter Park. ### What you'll do - Screen all referrals to the rehabilitation unit for appropriateness against CMS inpatient rehabilitation criteria. - Complete pre-admission screens capturing clinical data, insurance coverage, medical and rehabilitation needs, and discharge planning requirements. - Communicate admit or deny decisions promptly to referral sources, patients, families, and the broader healthcare team in collaboration with the rehab physician. - Evaluate each patient's equipment needs—oxygen therapy, IV therapy, durable medical equipment, specialty pharmaceuticals—and coordinate timely ordering for the inpatient unit. - Collaborate with Utilization Review, Case Management, Social Services, financial counselors, and insurance carriers to facilitate efficient patient transitions to the rehab unit. - Document patient interviews, evaluations, recommendations, and financial obligation notifications in the medical record. ### What we're looking for - Active Florida licensure as an RN, Physical Therapist, Speech-Language Pathologist, or Occupational Therapist (required). - Associate's degree (required); Bachelor's degree (preferred). - Current BLS/CPR certification (required). - 2+ years of experience in an acute care clinical or rehabilitation setting (preferred). - Ability to work a Monday–Friday 8 am–5 pm schedule with a rotating weekend assignment. ### What's included - Annual salary range: $65,582–$108,647. - Medical, dental, vision, life, and disability insurance effective Day One. - Paid time off from Day One. - 403(b) retirement plan. - Four weeks of 100% paid parental leave. - Pet benefits and whole-person well-being and mental health resources. ### About the practice AdventHealth is a faith-based, not-for-profit health system operating across multiple states with a mission rooted in whole-person care.
