Social Worker MSW Hospice Inpatient Unit

AdventHealth Home Health and Hospice is hiring a full-time MSW Social Worker for its Inpatient Hospice Unit (IPU) based in Orlando, Florida, with coverage at IPU sites in Altamonte Springs and Kissimmee. ### What you'll do - Evaluate psychosocial needs of hospice patients and families and develop individualized care plans. - Conduct initial assessments at patient residences and coordinate services with the interdisciplinary team. - Assist patients and families in securing financial resources and connecting with community referrals. - Educate patients and families on Advance Directives and support completion of those documents. - Provide individual and group counseling; assess and document bereavement needs from admission through the hospice process. - Coordinate with facility and hospital case managers and maintain accurate documentation in the electronic medical record. ### What we're looking for - Master's degree in social work, psychology, counseling, or a related field (required). - Licensed Social Worker (LSW) with applicable Florida state licensure (required). - Minimum 2 years of experience in the social work field (required); healthcare or hospice experience preferred. - Current BLS/CPR certification and valid driver's license (required). - Working knowledge of local, state, and federal entitlement programs and advanced directives. - Proficiency in electronic medical records and Microsoft Office (Word, Excel, Outlook, PowerPoint). ### What's included - Compensation range: $49,308–$91,705 per year. - Medical, dental, vision, life, and disability insurance effective Day One. - Paid time off from Day One and 4 weeks of 100% paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice AdventHealth Home Health and Hospice is a faith-based, mission-driven organization providing hospice and palliative services across the greater Orlando area.
