About this role
AdventHealth Home Health and Hospice is hiring a full-time Registered Nurse Care Manager for its hospice program based in Altamonte Springs, Florida, covering the Apopka area (zip codes 32712 and 32703).
### What you'll do
- Provide hospice nursing services to patients in their homes and in nursing facilities, managing an assigned caseload.
- Initiate and coordinate individualized Plans of Care, monitoring patient conditions and conducting regular assessments.
- Administer medications and treatments as prescribed; order necessary medications, supplies, and durable medical equipment.
- Educate patients, families, and caregivers on end-stage disease processes, symptom management, and safety.
- Collaborate with physicians and interdisciplinary team members to develop and adjust care plans based on symptomatic changes.
- Participate in on-call rotations for after-hours emergency clinical needs (approximately 2–3 call days per month, including some weekends).
### What we're looking for
- Active Registered Nurse (RN) license in Florida.
- Associate's Degree in Nursing required; Bachelor's Degree preferred.
- BLS/CPR certification and valid driver's license with current auto insurance required.
- 3+ years of experience as an RN, with home health and/or acute care background preferred.
- Certified Hospice and Palliative Nurse (CHPN) credential preferred.
- Proficiency with electronic medical records and Microsoft Office Suite; familiarity with InterQual criteria a plus.

### About the practice

This field-based role operates Monday through Friday, 8 a.m.–5 p.m., with a defined on-call commitment.
