Registered Nurse RN Admissions Hospice

AdventHealth Home Health and Hospice is hiring a full-time Registered Nurse for an Admissions Hospice role based in Altamonte Springs, Florida, covering Orange, Osceola, and Seminole Counties. ### What you'll do - Travel to patient locations and hospital rooms to assess physical and emotional condition for hospice eligibility - Partner with Account Representatives to manage new referrals and meet patient and hospital staff needs - Evaluate patients' medical needs, current medications, and special equipment requirements; obtain physician approval for program admission - Obtain required signatures and complete documentation to comply with reimbursement regulations; enter data into the electronic medical record promptly - Notify hospice team members for crisis intervention, initiate crisis care orders, and arrange patient transportation as needed - Communicate with pharmacy and applicable departments regarding patient equipment and medications; update referral sources on admission status ### What we're looking for - Active Florida Registered Nurse (RN) license - Minimum 3 years of RN experience in home health and/or acute care, including at least 1 year in a hospital setting - Associate's degree in Nursing required; Bachelor's degree preferred - Current BLS/CPR certification and valid Driver's License - Proficiency with electronic medical records and Microsoft Office Suite - Certified Hospice and Palliative Nurse (CHPN) credential preferred ### What's included - Salary range: $65,582–$108,647 per year - Sign-on bonus up to $10,000 - Medical, dental, vision, life, and disability insurance from day one - Paid time off from day one; 4 weeks fully paid parental leave - 403(b) retirement plan - Weekend bonus pay available for weekend schedule option ### About the practice AdventHealth Home Health and Hospice serves patients across Central Florida with a mission rooted in whole-person care.
