Registered Nurse RN Care Manager Hospice

An AdventHealth Home Health and Hospice RN Care Manager role serving the Winter Garden and Windermere area of Florida, providing hospice nursing services to patients in home and facility settings. ### What you'll do - Manage an assigned caseload of hospice patients, initiating and coordinating individualized Plans of Care - Conduct regular patient assessments, monitor conditions, and administer medications and treatments as prescribed - Educate patients, families, and caregivers on end-stage disease processes, symptom management, and safety - Collaborate with physicians and the interdisciplinary team to develop and adjust care plans and order medications, supplies, and equipment - Complete all required documentation accurately within the electronic medical records system - Participate in on-call rotations for after-hours emergency clinical needs (approximately 2–3 days per month, including some weekends) ### What we're looking for - Active Florida RN license (required) - Associate's Degree in Nursing (required); Bachelor's Degree in Nursing (preferred) - BLS/CPR certification and valid Driver's License (both required) - 3+ years of RN experience, preferably in home health or acute care settings - Certified Hospice and Palliative Nurse (CHPN) credential (preferred) - Proficiency with electronic medical records and Microsoft Office Suite ### What's included - Compensation range: $65,582–$108,647 annually - Medical, dental, vision, life, and disability insurance effective Day One - Paid Time Off from Day One and 4 weeks of 100% paid parental leave - 403(b) retirement plan - Career development resources and whole-person well-being support - Mental health resources and pet benefits ### About the practice AdventHealth Home Health and Hospice is part of AdventHealth, a faith-based, not-for-profit health system with a mission rooted in whole-person care — body, mind, and spirit.
