Quality Service Risk Coordinator MSW Hospice

AdventHealth Home Health and Hospice is hiring a PRN Quality Service Risk Coordinator (MSW) to support hospice patients and families across Seminole, Orange, and Osceola Counties in the Central Florida area. ### What you'll do - Evaluate psychosocial needs of patients and families and develop individualized care plans - Provide individual and group counseling to patients and families, including bereavement support throughout the hospice process - Travel to patient residences to conduct initial assessments and ongoing in-home evaluations - Coordinate referrals to community resources for financial and social needs; assist with documentation and benefit applications - Educate patients and families on Advance Directives and assist with completion - Collaborate in interdisciplinary team meetings and maintain accurate documentation in the electronic medical record ### What we're looking for - Master's degree in Social Work (MSW) required - Active Licensed Clinical Social Worker (LCSW) licensure required - Minimum 2 years of experience in the social work field - Healthcare or hospice experience preferred - BLS/CPR certification required - Proficiency with electronic medical record systems and knowledge of current counseling theories and practices ### What's included - Pay range: $41,096–$76,428 annually (PRN rate) - Hybrid field position covering Central Florida counties - Day shift schedule, Monday–Friday, 8:30 AM–5:00 PM ### About the practice AdventHealth Home Health and Hospice serves patients across Central Florida with a mission rooted in whole-person care — body, mind, and spirit.
