About this role
AdventHealth Home Health and Hospice is hiring a full-time Home Health Physical Therapy Assistant to deliver patient care across Hillsborough, Pasco, Pinellas, Polk, and Highlands/Hardee counties in Florida.
### What you'll do
- Implement physical therapy treatment plans established by a supervising Physical Therapist, applying evidence-based techniques in the patient's home.
- Monitor patient response to treatment and communicate findings to the Physical Therapist for plan adjustments.
- Document interventions and patient progress accurately and timely using measurable, objective terminology in accordance with insurance guidelines and department policies.
- Educate patients, family members, and caregivers on identified therapeutic needs and home exercise programs.
- Maintain productivity standards and adjust scheduling to meet patient needs across assigned counties.
- Participate in departmental meetings, education sessions, and performance improvement initiatives to sustain clinical competencies.
### What we're looking for
- Associate degree in Physical Therapy Assistant studies (required).
- Active PTA licensure (required).
- Current BLS/CPR certification (required).
- Valid driver's license to travel across assigned county territories (preferred).
- Basic computer proficiency including Microsoft Office.
- Prior home health experience preferred; entry-level candidates considered.

- Pay range: $23.71–$44.09/hr (~$49,316–$91,707 annualized).

The home health division supports interdisciplinary care teams focused on patient recovery, independence, and appropriate discharge planning. Staff operate Monday–Friday with a weekend on-call rotation.
