AdventHealth Home Health and Hospice is hiring a full-time Physical Therapist for a home health territory covering Clermont, Minneola, Mascotte, Groveland, and Leesburg, Florida, based out of Ocoee, FL.
### What you'll do
- Develop patient-specific care plans with measurable goals in collaboration with patients, families, and physicians, accounting for physical, cultural, psychosocial, and age-specific needs.
- Reassess patients at appropriate intervals, document progress, and evaluate the effectiveness of therapy interventions.
- Observe and record patient activities and findings, reporting changes or deviations in the plan of care to the supervising physician.
- Instruct patients and caregivers on the use of assistive devices and establish individualized home exercise programs.
- Document all treatments accurately and timely in the medical record, updating care plans as clinical needs evolve.
- Delegate tasks appropriately to support personnel based on skill level and professional scope.
### What we're looking for
- Active Physical Therapist (PT) licensure in Florida.
- Minimum 1 year of relevant clinical physical therapy experience.
- BLS/CPR certification (required at time of hire).
- Demonstrated knowledge of physical therapy theory, modalities, and evidence-based treatments.
- Familiarity with home care regulations and Medicare third-party reimbursement processes.
- Doctorate in Physical Therapy preferred; COSC (Oasis Specialist-Clinical) certification preferred.

- Hourly pay range: $34.71–$64.55 (approximately $72,196–$134,264 annualized).
