About this role

AdventHealth Hospice is hiring a full-time Registered Nurse Care Manager for field-based hospice care across multiple service areas in the greater Orlando, Florida region. ### What you'll do - Provide direct hospice nursing services to patients in their homes and in nursing facilities across an assigned geographic territory. - Initiate, coordinate, and adjust individualized Plans of Care for an assigned patient caseload. - Conduct regular patient assessments, monitor condition changes, and communicate findings and medication adjustments to physicians. - Administer medications and treatments as prescribed; order necessary supplies, equipment, and pharmacy resources. - Educate patients, families, and caregivers on end-stage disease processes, symptom management, and safety. - Participate in on-call rotations for after-hours emergency clinical needs (approximately 2–3 days per month, including some weekends). ### What we're looking for - Active Florida Registered Nurse (RN) license — required. - Associate's Degree in Nursing required; Bachelor's Degree preferred. - BLS/CPR certification — required. - Valid driver's license and personal auto insurance — required. - 3+ years of RN experience preferred, with home health and/or acute care background valued. - Certified Hospice and Palliative Nurse (CHPN) credential preferred. ### What's included - Salary range: $65,582–$108,647 annually. - Medical, dental, vision, life, and disability insurance effective Day One. - Paid time off from Day One; 4 weeks of 100% paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice AdventHealth Hospice is part of AdventHealth, a faith-based, not-for-profit health system with a mission rooted in whole-person care — body, mind, and spirit.
