About this role

A full-time Hospice Admissions RN role at UChicago Medicine AdventHealth serving patients across Lagrange, Hinsdale, Bolingbrook, and Glen Oaks hospitals and surrounding communities in Hinsdale, IL. ### What you'll do - Travel to hospital rooms and community sites to assess patient eligibility for hospice care - Partner with hospital case management teams to coordinate referrals and facilitate appropriate discharge planning - Evaluate patients' physical condition, medical history, current medications, and equipment needs to determine hospice appropriateness - Obtain physician orders and required signatures; generate electronic and paper documentation for regulatory compliance and reimbursement - Communicate care plan initiation and admission status updates to the hospice intake office, referral sources, and the interdisciplinary care team - Coordinate with Home Medical Equipment vendors and pharmacists regarding patient-specific equipment and medication needs ### What we're looking for - Active Illinois RN license (required) - Associate's degree in Nursing required; Bachelor's degree preferred - Minimum 1 year of acute care RN experience - Current BLS/CPR certification - Valid driver's license and personal auto insurance (field-based travel role) - Hospice or end-of-life care experience preferred ### What's included - Compensation: $33.60–$56.52/hr ($69,888–$117,562 annualized) - Medical, dental, vision, life, and disability insurance effective Day One - Paid time off starting Day One - 403(b) retirement plan - 4 weeks of 100% paid parental leave - Whole-person well-being and mental health support resources ### About the practice UChicago Medicine AdventHealth is a faith-based health system operating across the Chicago suburban corridor, including Hinsdale, Lagrange, Bolingbrook, and Glen Oaks campuses. The hospice admissions team works closely with hospital case management and physician staff to transition patients into appropriate end-of-life care programs. This role operates primarily Monday–Friday on a day shift schedule with weekend backup on a rotating quarterly basis.
