About this role

UChicago Medicine AdventHealth is hiring a full-time Certified Nursing Assistant (CNA) for its Hospice Fieldwork program, serving patients throughout the Greater Chicago / Great Lakes Region from its Hinsdale, IL base. ### What you'll do - Deliver hands-on personal care including bathing, oral hygiene, nail and foot care, and assistance with ADLs in patients' homes, SNFs, and assisted living facilities. - Support patient mobility and perform range-of-motion activities as outlined in each individualized plan of care. - Prepare and serve meals or snacks and assist patients with eating and drinking as needed. - Communicate with patients and family members to assess living conditions, ensure safety, and respond to emotionally difficult situations. - Document each patient visit and activities performed in the electronic medical record, flagging needs for other disciplines when appropriate. - Participate in team meetings, in-service training, and department activities to support coordinated hospice care. ### What we're looking for - Active Certified Nurse Assistant (CNA) licensure — required. - Valid driver's license and personal auto insurance — required (fieldwork role with travel). - BLS/CPR certification — required. - High school diploma or equivalent — required. - Proficiency with electronic medical records and written/verbal English communication — required. - Certified Hospice and Palliative Nurse (CHPN) credential — preferred. ### What's included - Pay range: $17.17–$27.46/hr (~$35,714–$57,117 annualized). - Medical, dental, vision, life, and disability insurance effective Day One. - Paid time off from Day One. - 403(b) retirement plan. - Four weeks of fully paid parental leave. - Mental health resources and whole-person well-being support. ### About the practice UChicago Medicine AdventHealth Hinsdale is a faith-based health system committed to whole-person care — body, mind, and spirit.
