About this role
AdventHealth Home Health and Hospice is hiring a full-time Licensed Practical Nurse (LPN) to deliver in-home patient care across the Lake County area in Tavares, Florida.
### What you'll do
- Perform home visits providing direct patient care including medication administration, wound dressing changes, and ostomy care under RN direction.
- Assess patients during visits, document vital signs, and identify signs of abuse or neglect using age-specific guidelines.
- Receive, verify, and implement physician orders, ensuring co-signature by an RN as required.
- Report pertinent clinical observations to the attending physician and RN and implement care plan changes as directed.
- Perform diagnostic testing including EKG, spirometry, audiometry, pulse oximetry, and forensic specimen collection.
- Participate in weekend and after-hours call rotation with flexible scheduling Monday–Friday, 8am–5pm.
### What we're looking for
- Active Florida Licensed Practical Nurse (LPN) license.
- Current BLS/CPR certification.
- Valid driver's license and personal auto insurance required for home visit travel.
- High school diploma or equivalent plus completion of an accredited LPN vocational program.
- 1+ year of medical/surgical nursing experience preferred.
- IV certification preferred; venipuncture skills required.
