About this role
AdventHealth Hospice is hiring a part-time Licensed Clinical Social Worker (MSW) to provide psychosocial support to hospice patients and families across Adams, Arapahoe, Jefferson, Denver, Douglas, Elbert, and Elizabeth Counties in Colorado.
### What you'll do
- Assess patients' physical, psychosocial, cultural, and spiritual needs through observation, interview, and medical record review
- Document ongoing patient status, interventions, responses, and outcomes accurately and in a timely manner
- Match clinical competencies to individualized patient needs and manage resource and patient flow
- Apply standards of care, policies, procedures, and regulatory guidelines to guide practice
- Participate in quality improvement, evidence-based practice, and research activities at the unit level
- Demonstrate working knowledge of the involuntary commitment process as applicable to hospice patients
### What we're looking for
- Master's degree in Social Work (MSW) required
- Active Licensed Clinical Social Worker (LCSW) licensure in Colorado required
- Minimum 1 year of experience in a hospital or clinical setting
- Strong organizational, verbal interpersonal, and customer relations skills
- Basic Life Support (BLS/CPR) certification preferred
- Ability to travel across seven Colorado counties for patient visits

AdventHealth Hospice is part of AdventHealth, a faith-based, not-for-profit health system committed to whole-person care in body, mind, and spirit. The hospice program serves patients and families across a broad multi-county region in the Denver metro area. This part-time role follows a daytime schedule (8:30 AM–5:00 PM, 20 hours/week) with flexible day options after a two-week Monday–Friday orientation.
