About this role
AdventHealth Home Health and Hospice is hiring a full-time Physical Therapist Care Manager to deliver home-based physical therapy services in the Palm Coast/Flagler area of Ormond Beach, Florida.
### What you'll do
- Develop patient-specific care plans with measurable goals in collaboration with patients, families, and physicians, accounting for physical, cultural, psychosocial, and spiritual needs.
- Reassess patients at appropriate intervals, document progress, and evaluate the effectiveness of therapy interventions.
- Observe and record patient activities and findings, reporting changes or deviations in the plan of care to the supervising physician.
- Instruct patients and caregivers on the use of assistive devices and establish individualized home exercise programs.
- Delegate tasks to appropriate personnel based on skill level and professional standing.
- Apply knowledge of home care regulations and Medicare third-party reimbursement as they impact care delivery.
### What we're looking for
- Active Physical Therapist (PT) license — required.
- Minimum 1 year of relevant clinical physical therapy experience — required.
- BLS/CPR certification — required.
- Doctorate in Physical Therapy — preferred.
- Recent experience in a Medicare-certified home health agency in a care-manager role — preferred.
- Certified OASIS Specialist-Clinical (COSC) and valid Driver's License — preferred.

The home health division serves patients in their residences across Florida, operating under Medicare-certified standards with a multidisciplinary care team model. This role covers the Palm Coast/Flagler service area on a Monday–Friday daytime schedule with flexible hours and rotating weekend call.
