About this role

A Referral Specialist at AdventHealth (Hospice of the Comforter) coordinates patient admissions and referral intake at the Wesley Chapel, Florida facility. ### What you'll do - Initiate patient records in the EMR, documenting demographics, condition, referral source, and location upon intake - Provide telephonic support to referral sources and schedule nurse visits to facilitate patient admissions - Create and distribute daily triage reports summarizing new admissions and referral outcomes - Communicate with clinical staff to relay patient updates, order requested equipment or supplies, and dispatch clinical providers for assessments - Monitor scheduling systems and maintain the pending admissions log to ensure timely, accurate appointments - Obtain patient/family consents and relay relevant information as needed throughout the admission process ### What we're looking for - High school diploma or equivalent required; Associate degree preferred - 3+ years of administrative, clerical, healthcare, or call-center experience - Proficiency with electronic medical records and Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) - Working knowledge of medical terminology - Strong written and verbal English communication skills - Ability to think critically and manage multiple priorities within the admissions workflow ### What's included - Pay range: $16.63–$26.60/hr ($34,590–$55,328 annualized) - Medical, dental, vision, life, and disability insurance from day one - Paid time off from day one - 403(b) retirement plan - Four weeks of fully paid parental leave - Whole-person well-being and mental health resources ### About the practice AdventHealth is a faith-based, not-for-profit health system operating across multiple states with a mission centered on whole-person care.
