About this role
AdventHealth Winter Park is hiring a full-time Care Management Social Worker to support inpatient, observation, and emergency department patients in Winter Park, Florida.
### What you'll do
- Provide grief counseling, disease adjustment support, crisis intervention, and goals-of-care planning for patients and families.
- Conduct holistic assessments of discharge planning needs, including prior functioning, support systems, financial status, and psychosocial factors.
- Review medical records — including medications, history and physical, labs, and progress notes — to integrate clinical, social, and financial factors into transition-of-care plans.
- Develop and update discharge plans with contingency options throughout the hospital stay to ensure timely care coordination and progression.
- Coordinate post-acute care placements and community-based services to address social determinants of health.
- Communicate patient care information to post-acute facilities using standard work and technology platforms to ensure continuity and accurate discharge reconciliation.
### What we're looking for
- Master of Social Work (MSW) required; or Bachelor of Social Work (BSW) with a minimum of 3 years of hospital or medical social work experience.
- 3+ years of social work experience required.
- 3+ years of care management experience preferred.
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) credential preferred.
- Availability for a Tuesday–Saturday day-shift schedule.
