About this role

AdventHealth Home Health and Hospice is hiring a full-time Occupational Therapy Care Manager to deliver home-based occupational therapy services across Volusia and Flagler Counties in the Ormond Beach, Florida area. ### What you'll do - Conduct comprehensive occupational therapy evaluations assessing functional abilities, ADLs, and home environment safety for home health patients. - Develop and implement patient-specific care plans with measurable goals in collaboration with patients, families, and physicians. - Provide direct patient care, delegate tasks to appropriate team members, and supervise the home health care team. - Educate patients and caregivers on therapy techniques, assistive device use, and home exercise programs. - Monitor and document patient progress, adjusting care plans as needed to achieve optimal outcomes. - Participate in interdisciplinary care conferences and quality improvement activities. ### What we're looking for - Active Occupational Therapist (OT) licensure in Florida (required). - Minimum 1 year of relevant clinical occupational therapy experience (required). - Master's degree in Occupational Therapy (preferred). - Recent experience in a Medicare-certified home health agency as a case manager (preferred). - BLS/CPR certification (required). - Valid driver's license and ability to perform home visits throughout Volusia and/or Flagler Counties. ### What's included - Pay range: $31.55–$58.69/hour ($65,624–$122,075 annualized). - Medical, dental, vision, life, and disability insurance effective Day One. - Paid Time Off from Day One. - 403(b) retirement plan. - Four weeks of fully paid parental leave. - Mental health resources, whole-person well-being programs, and pet benefits. ### About the practice AdventHealth Home Health and Hospice is part of AdventHealth, a faith-based, not-for-profit health system operating across multiple states. The home health division supports patients recovering in their own homes across Volusia and Flagler Counties, coordinating care through a collaborative interdisciplinary team model. The role operates Monday through Friday, 8 am–5 pm, with flexible scheduling, rotating weekends, and some on-call responsibility.
