- Conducts complex psychosocial assessments and targeted interventions to promote timely throughput, safe discharge, and prevention of avoidable readmissions
- Identifies and assesses patients at risk for readmission; develops and executes individualized transition management plans
- Provides education to physicians, patients, families, and caregivers as part of a structured care coordination model
- Practice governed by national case management standards, TJC accreditation requirements, and state and federal regulatory compliance

Qualifications
- Graduate of an accredited School of Social Work with a Bachelor of Social Work (BSW) required; Master of Social Work (MSW) preferred
- Current Texas State Social Work License commensurate with degree level required
- Accredited Case Manager (ACM) certification preferred
- No minimum experience required; two or more years of acute care experience strongly preferred
- Demonstrated competency in psychosocial assessment, transition planning, and resource coordination in a hospital or health system setting
