A Quality & Value Clinical Specialist (LPN or CMA) role at AdventHealth supporting population health and value-based care operations at their Maitland, Florida office.
### What you'll do
- Conduct pre-visit chart reviews to identify open orders, care gaps, HCC coding opportunities, medication needs, and overdue wellness visits
- Outreach patients to complete health risk assessments, reconcile medications, assess social determinants of health, and schedule or reschedule appointments
- Coordinate transitional care management following ED or hospital discharges, including LACE score calculation, medication reconciliation, and symptom review
- Abstract records and notify the health plan of documentation for HEDIS care gap closure and CMS Star measure programs
- Connect patients and practices to pharmacy, care management, and external resources to address barriers to care
- Monitor staff productivity and quality of output, provide team training, and participate in meetings with payers and cross-functional teams
### What we're looking for
- Active Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) credential — required
- Minimum 3 years of healthcare experience — required
- Proficiency with Epic or another EMR platform — required
- Knowledge of chronic and acute disease management and value-based care concepts — required
- Familiarity with HEDIS, CMS Star ratings, CDI, RAF, and HCC coding — preferred
- CPC, CCS, or COC certification — preferred
