Practice Highlights
- Primary oversight of the facility-wide patient safety program — full programmatic authority and accountability
- Leads and attends Root Cause Analysis (RCA), Focused Reviews, and Intense Analysis sessions
- Directs process improvement initiatives targeting reduction of medical errors and unintended adverse patient outcomes
- Coordinates the activities of the patient safety function and/or patient safety committee
- Develops, revises, and maintains policies and procedures governing patient safety across the facility
- Oversees annual creation, review, and refinement of the Patient Safety Program scope
- Ensures compliance with audits and monitoring requirements as defined in corrective action plans
- Interfaces directly with Risk Management on potential risk issues and escalation pathways
- Educates clinical staff and practitioners on systems-based causes of medical errors and evidence-based safety strategies

Qualifications
- Bachelor's degree in a patient care-related field required; Master's degree in Nursing, Health Services, or Patient Safety — or a J.D. — strongly preferred
- Completion of a patient safety curriculum through ASHRM, NPSF, AHA, IHI, or an equivalent accredited academic provider
- Minimum 5 years of direct patient care experience required
- Minimum 3 years of experience in Quality, Risk Management, or Patient Safety required
- CPHRM (Certified Professional in Healthcare Risk Management) or CPPS (Certified Professional in Patient Safety) certification required, or eligibility and commitment to sit for the exam within two years of hire
- Current BLS certification required
