Practice Highlights
- Inpatient-only Palliative Care role — no outpatient panel, no call from home for routine matters
- Consultative scope covering patients with serious or life-limiting illness across inpatient service lines
- Lead goals-of-care conversations and family meetings, including complex prognostic discussions
- Collaborate daily with an interdisciplinary team: nursing, social work, chaplaincy, and case management
- Manage care transitions, including coordination of hospice referrals and discharge planning

Qualifications
- Board certification as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Active Texas NP or PA license, or demonstrated ability to obtain prior to start
- Active DEA certification required
- Prior palliative care, serious illness, or hospice experience strongly aligned with this role
- Demonstrated skill in goals-of-care communication and family-centered care delivery
