About this role

AdventHealth Home Health and Hospice is hiring a full-time Licensed Practical Nurse (LPN) to deliver home-based patient care across West Volusia County — including Deltona, DeLand, and surrounding areas — in Ormond Beach, Florida. ### What you'll do - Provide direct patient care including medication administration, wound dressing changes, and ostomy care under RN and physician direction - Perform patient assessments, document vital signs, and identify signs of abuse or neglect using age-specific guidelines - Receive, verify, and implement physician orders, ensuring co-signature by an RN as required - Conduct diagnostic testing including EKG, spirometry, audiometry, pulse oximetry, and forensic specimen collection - Communicate patient status changes to the attending physician, RN, and home health care team - Participate in weekend/after-hours coverage and call rotation as scheduled ### What we're looking for - Active Florida LPN license (required) - BLS/CPR certification (required) - Graduate of an accredited school of licensed practical nursing (required) - 1+ year of medical/surgical experience (preferred) - IV certification (preferred) - Valid driver's license and reliable transportation for home visits (preferred) ### What's included - Sign-on bonus up to $2,000 - Hourly pay range: $20.97–$38.99 (approx. $43,618–$81,099 annually) - Medical, dental, vision, life, and disability insurance from day one - Paid time off from day one - 403(b) retirement plan - Four weeks of 100% paid parental leave - CME and career development resources ### About the practice AdventHealth Home Health and Hospice is a faith-based health system serving patients in their homes across Florida, focused on whole-person care in body, mind, and spirit. This role covers a defined West Volusia County territory with a consistent Monday–Friday daytime schedule and rotating weekend coverage. The team operates with a collaborative model pairing LPNs directly with RN supervisors and attending physicians.
