Licensed Practical Nurse LPN Home Health

A full-time Licensed Practical Nurse (LPN) provides home health visits for AdventHealth Home Health and Hospice Inc in Ormond Beach, Florida, covering South Volusia County including Port Orange and New Smyrna. ### What you'll do - Provide direct patient care — wound dressing changes, ostomy care, medication administration, and treatments — under RN and physician direction - Perform diagnostic testing including EKG, spirometry, audiometry, pulse oximetry, and forensic collection in accordance with established guidelines - Assess patients at each visit, documenting vital signs, medical information, and screening for signs of abuse or neglect using age-specific guidelines - Receive, verify, and implement physician orders, ensuring LPN-taken orders are co-signed by an RN - Communicate pertinent observations and care-plan changes to the attending physician, RN, and the broader home health team - Participate in weekend and after-hours call rotation; maintain clinical records within established time frames ### What we're looking for - Active Florida Licensed Practical Nurse (LPN) license - Current BLS/CPR certification - Graduate of an accredited school of licensed practical nursing - Valid driver's license (required for home visit territory coverage) - 1+ year of medical/surgical experience (preferred) - IV certification and Point of Care documentation software experience (preferred) ### What's included - Sign-on bonus up to $2,000 - Hourly pay range $20.97–$38.99 (~$43,618–$81,099 annualized) - Medical, dental, vision, life, and disability insurance from Day One - Paid Time Off from Day One - 403(b) retirement plan - 4 weeks of 100% paid parental leave ### About the practice AdventHealth Home Health and Hospice is part of AdventHealth, a faith-based, not-for-profit health system operating across multiple states.
