<p>Team Florida is recruiting a Primary Care Physician to lead a dedicated senior care team in Westchester, a Miami Dade community with one of South Florida's largest concentrations of older adults. This is a value-based practice model built around quality outcomes rather than visit volume—you'll manage a smaller, focused patient panel with longer visits, supported by a full multidisciplinary team including APPs, nurses, medical assistants, pharmacists, behavioral health specialists, and care coordinators. The role offers the opportunity to shape senior primary care delivery for a vibrant, deeply rooted, largely Spanish-speaking community as part of Team Florida's statewide network spanning 93 clinics across 53 Florida communities.</p>

<h3>Qualifications</h3> <ul> <li>Active, unrestricted Florida medical license (MD or DO)</li> <li>Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine</li> <li>Active DEA registration for prescribing</li> <li>Experience or demonstrated strong interest in senior-focused primary care and value-based, team-based care delivery</li> <li>Comfort managing a smaller patient panel with longer visit times and a proactive, preventive care philosophy</li> <li>Strong communication and relationship-building skills; bilingual English and Spanish preferred</li> </ul>
