This is a value-based practice model built around quality outcomes rather than visit volume—you'll manage a smaller, focused patient panel with longer visits, supported by a full multidisciplinary team including APPs, nurses, medical assistants, pharmacists, behavioral health specialists, and care coordinators. The role offers the opportunity to shape senior primary care delivery for a vibrant, deeply rooted, largely Spanish-speaking community as part of Team Florida's statewide network spanning 93 clinics across 53 Florida communities.</p>
