Team Florida is recruiting a Primary Care Physician to lead senior-focused care at its Seffner clinic, serving the Tampa East and South market. This is a value-based position built around a deliberately smaller patient panel, extended visit times, and a full embedded care team—nurses, medical assistants, care coordinators, and behavioral health support—designed to enable proactive, preventive medicine rather than reactive volume-driven care. You will manage your own dedicated panel from a single clinic location, with direct input into practice-level quality and outcomes improvement as the organization stands up a new statewide senior primary care model.

Practice Highlights

Senior-focused patient population managed through a dedicated panel model

Extended visit times built for complex care planning, relationship building, and shared decision-making

Proactive and preventive care focus including chronic disease management, annual wellness visits, and early intervention to reduce hospitalizations

Daily collaboration with in-clinic nurses, medical assistants, and care coordinators to close care gaps

Regular review of panel-level quality and outcomes data to guide care plan adjustments and reduce avoidable hospital admissions

Care coordination with specialists, hospitalists, and skilled nursing partners to ensure smooth transitions of care

Single-clinic practice location in Seffner with full-time availability required

Mentorship and collaboration with advanced practice clinicians and support staff within the clinic
