<h3>Practice Highlights</h3> <ul> <li>Serves the highest-risk health plan members within the Senior Community Care (SCC) integrated care delivery program</li> <li>Practice settings span patients' homes, skilled nursing facilities, nursing homes, and assisted living / senior housing communities across AL, FL, and MS</li> <li>Approximately 85% travel across the tri-state region; home base is flexible within those states</li> <li>Conducts comprehensive assessments including past medical history review, medication reconciliation, vital signs, physical examination, evidence-based screenings, and point-of-care testing</li> <li>Supports members through short-term sub-acute rehabilitation episodes</li> <li>Leads advanced illness and advanced care planning conversations with patients and caregivers</li> <li>Provides patient and caregiver counseling and education as a core clinical function</li> <li>Ensures accurate ICD-10 documentation and complete clinical records in an electronic medical record platform</li> <li>Collaborates within an interdisciplinary care team model, coordinating across the full care continuum</li> <li>Part of a nationally scaled program that is among the largest employers of Nurse Practitioners in the United States, with defined pathways for professional development and role advancement</li> </ul>

<h3>Qualifications</h3> <ul> <li>For NPs: Active ANCC or AANP board certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care</li> <li>For PAs: Current NCCPA certification with active state licensure</li> <li>Active, unrestricted licensure in the state(s) of practice — or ability to obtain prior to start date — with no current disciplinary actions or practice limitations</li> <li>Active and unrestricted driver's license plus access to reliable personal transportation (required for field-based travel)</li> <li>Active DEA licensure and prescriptive authority, or ability to obtain post-hire per applicable state regulations</li> <li>Ability to obtain a collaborative practice agreement where required by state law</li> <li>Physical ability to navigate varied care environments and position oneself as needed to perform clinical duties</li> <li>Preferred: 1+ years of clinical APC experience, ideally in long-term care, geriatric medicine, senior living, or home care</li> <li>Preferred: Experience managing patients with complex behavioral, social, and/or functional needs; familiarity with advanced illness and end-of-life care frameworks</li> <li>Preferred: Proficiency with electronic medical records and clinical technology platforms</li> </ul>
