About this role
A part-time RN Care Manager role on weekends at AdventHealth Altamonte Springs, a faith-based health system in Altamonte Springs, Florida.
### What you'll do
- Participate in multidisciplinary rounds to review patient status, progression of care, and discharge planning for all assigned patients.
- Assess patients and families holistically — including prior functioning, support systems, financial, and psychosocial factors — to develop timely discharge plans.
- Communicate with payers to obtain authorizations for post-acute care services and coordinate arrangements for post-discharge needs.
- Facilitate patient and family care conferences with the multidisciplinary team and advocate for patient empowerment in healthcare decisions.
- Document discharge planning evaluations, MDR notes, barriers to progression, avoidable days, and patient needs in the electronic medical record per standard work.
- Evaluate readmitted patients to identify perceived reasons for readmission and adjust care coordination plans accordingly.
### What we're looking for
- Active Florida RN license (required).
- Associate's degree in Nursing required; Bachelor's degree preferred.
- Minimum 2 years of medical/hospital nursing experience.
- Prior Care Management or Utilization Management experience preferred.
- CCM or ACM certification preferred; BLS preferred.
- Proficiency with electronic medical records and strong critical thinking, conflict resolution, and organizational skills.
