<p>Team Florida is recruiting an experienced Hospitalist to provide inpatient medicine for senior patients across Florida's partner hospital network. This is a per diem role built on a value-based model where clinical judgment and patient outcomes—not encounter volume—drive your work. You will round on Team Florida's established senior patient population, coordinate directly with their outpatient primary care physicians, and shape how inpatient and primary care integrate across the state.</p>

<h3>Practice Highlights</h3> <ul> <li>Inpatient census limited by design to allow adequate time per patient and comprehensive clinical assessment</li> <li>Patient population: established senior Medicare Advantage and value-based care cohort with continuity to outpatient primary care</li> <li>Daily responsibilities include rounding on admitted patients, developing individualized treatment plans, coordinating with hospital case managers and discharge planners, and ensuring safe transitions to home or post-acute care</li> <li>Direct communication with each patient's outpatient primary care physician and care team throughout the hospital stay</li> <li>Discharge planning and handoff: clear documentation, personal communication with primary care teams, and identification of care gaps</li> <li>Focus on quality outcomes including readmission prevention and care continuity</li> <li>Flexible regional assignment within Florida's partner hospital network to match your availability and preferences</li> </ul>

<h3>Qualifications</h3> <ul> <li>MD or DO from an accredited medical school</li> <li>Active, unrestricted Florida medical license</li> <li>Board certification or board eligibility in Internal Medicine, Family Medicine, or a related specialty</li> <li>Active DEA registration</li> <li>Current hospital privileges or ability to obtain privileges at Team Florida partner hospitals</li> <li>Experience caring for older adults, preferably within a Medicare Advantage or value-based care setting</li> <li>Current BLS certification; ACLS as required by individual facilities</li> <li>Strong communication and care coordination skills with primary care physicians and interdisciplinary teams</li> </ul>
