RN Care Management PRN

An RN Care Management PRN role at AdventHealth Wesley Chapel in Wesley Chapel, Florida, supporting inpatient care coordination and discharge planning across the continuum of care. ### What you'll do - Develop individualized care plans that integrate patient and family preferences and values - Monitor patient care through ongoing assessments, evaluations, and medical record review - Advocate for patient resources and work to remove barriers to effective care delivery - Maintain active communication with supervisors and interdisciplinary care team members to support health plan implementation - Serve as a resource for medical record documentation accuracy and appropriateness of services relative to diagnoses - Coordinate and evaluate treatment options for post-discharge and transitional care ### What we're looking for - Active Registered Nurse (RN) license in Florida (required) - Associate's Degree in Nursing (required); Bachelor's Degree in Nursing (preferred) - Basic Life Support (BLS/CPR) certification (preferred) - Ability to work day shifts on a PRN basis ### What's included - Pay range: $32.76–$57.47/hour (~$68,141–$119,538 annualized) - PRN scheduling flexibility on day shift - Employment with AdventHealth, a large faith-based nonprofit health system ### About the practice AdventHealth Wesley Chapel is part of AdventHealth, a faith-based nonprofit health system with facilities across multiple states.
