AdventHealth is hiring a Senior Manager of Care Management Social Work at its Lakeview Plaza facility in Altamonte Springs, Florida.
### What you'll do
- Manage daily operations of the care management department across inpatient, observation, and emergency department settings.
- Lead interdisciplinary rounds and family/care team meetings to address discharge barriers and ensure timely patient transitions.
- Develop and implement effective discharge plans in collaboration with patients, families, care teams, and payors.
- Monitor dashboards and reports to track staff compliance with documentation and regulatory requirements.
- Hire, onboard, educate, and retain care management staff while coaching them on accurate documentation and best practices.
- Collaborate with outside agencies and the management team to reduce length of stay, lower avoidable readmissions, and improve patient experience.
### What we're looking for
- Master's in Social Work, or a BSW plus a Master's in a related field (required).
- Minimum 4 years of direct care management experience (required).
- Minimum 2 years of social work experience (required).
- Strong knowledge of CMS Conditions of Participation, medical necessity criteria, and managed care/value-based programs.
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification (preferred).
- Hospital care management leadership experience (preferred).

- Compensation range: $79,403–$147,697 annually.
