Care Management Assistant

AdventHealth DeLand is hiring a PRN Care Management Assistant to support inpatient care coordination at their DeLand, Florida campus. ### What you'll do - Coordinate patient care and service arrangements under the direction of an RN or Social Work Care Manager - Deliver and document regulatory letters to patients and representatives per federal and state requirements - Arrange patient transportation and follow-up appointments as requested - Obtain insurance authorizations for post-acute care needs as directed by the Care Manager - Collaborate with care managers, physicians, and the broader healthcare team to support transitions of care ### What we're looking for - High school diploma or equivalent (required) - Intermediate to advanced proficiency in Microsoft Office (Word, Outlook, PowerPoint, Excel) - Strong interpersonal and written/verbal communication skills - 1+ year of office experience, preferably in a medical setting (preferred) - Previous data entry experience (preferred) - Familiarity with medical terminology (helpful) ### What's included - Pay range: $17.11–$27.38/hour ($35,589–$56,950 annualized) - Weekend day shift schedule (PRN) ### About the practice AdventHealth DeLand is part of AdventHealth, a faith-based not-for-profit health system operating across multiple states.
