About this role
A Care Manager RN at AdventHealth coordinates discharge planning and care transitions for inpatient, observation, and emergency department patients at the community clinic on Kurt St in Eustis, Florida.
### What you'll do
- Conduct holistic assessments of patients and families for discharge planning needs, incorporating clinical, social, financial, and psychosocial factors.
- Develop and maintain discharge and contingency plans throughout the hospital stay, arranging post-acute care services and community resources.
- Participate in multidisciplinary rounds to review patient status, level of care, and discharge plans, escalating care delays to leadership as needed.
- Communicate with payors to obtain authorization for post-acute care services and coordinate with facilities to ensure continuity of care.
- Facilitate patient and family care conferences and educate patients on the emotional, social, and financial impacts of illness.
- Document discharge planning evaluations, MDR participation, barriers to progression, and avoidable days per standard work protocols.
### What we're looking for
- Active Registered Nurse (RN) license required.
- Associate's Degree in Nursing required; Bachelor's Degree preferred.
- Minimum 1 year of nursing experience required.
- Valid Driver's License required.
- Basic Life Support (BLS) certification preferred.
- Ability to review medical records and integrate clinical, social, and financial data into transition-of-care planning.
