About this role

AdventHealth Waterman is seeking a full-time Registered Nurse for its Community Care and Transitional Care Programs in Tavares, Florida. ### What you'll do - Collaborate with the care team to develop, implement, and modify individualized patient care plans based on assessment findings and expected outcomes. - Conduct medication reconciliation and provide direct patient care coordination across community and transitional care settings. - Perform patient assessments addressing medical, behavioral, psychological, and emotional needs. - Document patient visits accurately and ensure timely communication with patients, families, and other healthcare providers. - Support community outreach educational programs, including development and distribution of patient education materials. - Provide care coordination coverage at additional regional locations as needed. ### What we're looking for - Active Registered Nurse (RN) license in Florida. - Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred. - BLS/CPR certification required. - Valid Driver's License required. - 3+ years of healthcare experience preferred. - Computer proficiency and working knowledge of mind-body medicine and current related research. ### What's included - Salary range: $65,582–$108,647 annually. - Medical, dental, vision, life, and disability insurance effective Day One. - Paid Time Off from Day One. - 403(b) retirement plan. - Four weeks of fully paid parental leave. - Career development resources and whole-person well-being support. ### About the practice AdventHealth Waterman is a faith-based, not-for-profit health system located in Tavares, Florida, serving Lake County and surrounding communities.
