About this role
AdventHealth Parker is hiring a full-time Care Manager RN to coordinate inpatient discharge planning and care transitions at its hospital in Parker, Colorado.
### What you'll do
- Participate in multidisciplinary rounds to review patient status, progression of care, and discharge plans, escalating care delays to leadership as needed.
- Assess patients and families holistically for discharge planning needs — including prior functioning, support systems, financial factors, and psychosocial needs — across inpatient, observation, and emergency settings.
- Develop and maintain discharge plans with contingency options throughout the hospital stay, arranging post-acute care services and community resources.
- Communicate with payors to obtain authorization for post-acute care and coordinate medical records for continuity of care.
- Organize and facilitate patient and family care conferences with the multidisciplinary team.
- Document discharge planning evaluations, ongoing assessments, barriers to progression, avoidable days, and MDR notes per standard work.
### What we're looking for
- Active Registered Nurse (RN) license required.
- Associate's degree in Nursing required; Bachelor's degree preferred.
- Minimum 2 years of medical/surgical hospital nursing experience required.
- Prior care management or utilization management experience preferred.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred.
- BLS certification preferred.
