<h3>Qualifications</h3> <ul> <li>Active Registered Nurse (RN) licensure required; Florida license or eligibility to obtain</li> <li>Minimum Associate Degree in Nursing or equivalent certificate program</li> <li>At least 2 years of recent acute care experience (within the last 3 years) — ICU, CCU, Med-Surg, LTAC, or direct Case Management experience strongly preferred</li> <li>Demonstrated ability to conduct psychosocial assessments, develop individualized care plans, and coordinate discharge planning across an interdisciplinary team</li> <li>Familiarity with Medicare and commercial insurance benefit structures as they apply to rehabilitation and acute care settings</li> <li>Must successfully complete a state-mandated criminal record check as a condition of employment</li> </ul>
