About this role

AdventHealth is hiring a Certified Medical Assistant (Health Coach) for its AH Well 65+ outpatient clinic in Eustis, Florida. ### What you'll do - Coordinate care across the continuum, including scheduling appointments and managing referrals for senior members - Perform initial health screens and intake assessments for new members - Obtain and review prior medical records to build a comprehensive patient history - Support daily clinic operations including phone triage, front desk assistance, and maintaining the clinical space - Communicate with members and care teams through digital channels to facilitate ongoing care - Manage each member's care plan in collaboration with the RN Case Manager and supervising provider ### What we're looking for - Graduate of an accredited Medical Assistant program (Technical/Vocational school required) - Active certification required: CMA, CMAC, RMA, CCMA, NCMA, or NRCMA - Current BLS/CPR certification - Knowledge of chronic disease management and care navigation - Strong patient advocacy skills with ability to navigate complex health systems - New graduates considered; no prior work experience required ### What's included - Pay range: $17.11–$27.38/hr (~$35,589–$56,950 annualized) - $2,000 sign-on bonus for eligible candidates - Medical, dental, vision, life, and disability insurance from day one - Paid time off from day one - 403(b) retirement plan - 4 weeks of 100% paid parental leave - CME and career development resources ### About the practice AdventHealth Well 65+ in Eustis, FL is a dedicated outpatient care model designed exclusively for adults 65 and older, focusing on whole-person wellness and proactive chronic disease management. The program operates Monday–Friday, 8:00 am–5:00 pm, within AdventHealth's broader faith-based health system spanning multiple states. The care team model pairs medical assistants closely with RN Case Managers and providers to deliver coordinated, relationship-centered care.
