About this role
Ascension Medical Group is hiring a Geriatrics Physician to join its PACE (Program of All-inclusive Care for the Elderly) program at Ascension Living PACE Michigan in Flint, MI.
### What you'll do
- Evaluate and treat a geriatric patient population within the PACE program model
- Coordinate care across interdisciplinary teams to address complex medical and social needs
- Develop and implement individualized care plans focused on improving patient quality of life
- Monitor ongoing health status and adjust treatment plans as conditions evolve
- Collaborate with nursing, social work, therapy, and community service providers
### What we're looking for
- MD or DO with board certification or eligibility in Geriatric Medicine or Internal Medicine/Family Medicine with geriatrics focus
- Active Michigan medical license or ability to obtain prior to start
- Experience evaluating and managing older adults with complex, chronic conditions
- Commitment to patient-centered, interdisciplinary care
- Familiarity with PACE or similar community-based care models preferred
