Nacogdoches Medical Center, a nearly 50-year anchor of East Texas healthcare, is recruiting a full-time clinical pharmacist for its day-shift inpatient pharmacy. This is a hands-on, high-scope role embedded in a full-service acute care hospital with robust service lines — cardiology with cath lab and electrophysiology, oncology, neuroscience with Brainlab technology, OB/GYN, and bariatric surgery. Pharmacists here function as true clinical partners to the medical and nursing teams, with collaborative practice authority across anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside protocols.

Practice Highlights

  • Full-time, day-shift schedule at an acute care community hospital serving East Texas for nearly five decades
  • Clinical scope includes order verification, medication profile review, drug-drug and drug-food interaction screening, allergy review, dosing appropriateness, and duplication checks
  • High-risk IV review, renal dose adjustment, IV-to-PO conversions, and black box warning interventions are core daily responsibilities
  • Collaborative practice management under facility-approved protocols: anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults
  • Active role in medication reconciliation and clarification of unsafe or unclear drug therapy orders
  • Oversight and supervision of pharmacy technicians and support staff throughout the preparation, labeling, and dispensing process
  • Participation in education and training of physicians, nurses, pharmacy technicians, and students
  • Regulatory compliance responsibilities including nursing unit inspections and adherence to Joint Commission, federal, and state standards
  • Service lines supported: cardiology (cath lab, electrophysiology), cancer care, neuroscience (Brainlab, stroke), OB/GYN, and weight loss surgery
  • Ongoing interface with medical and nursing staff on pharmaceutical supplies, drug compatibility, and drug utilization

Qualifications

  • Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy required
  • Active Texas pharmacy license required
  • AHA BLS required; ACLS and PALS required if role includes direct patient care responsibilities
  • Hospital pharmacy experience preferred
  • Board Certification in Pharmacotherapy (BCPS), Certified Geriatric Pharmacist (CGP), or equivalent specialty certification preferred
  • Physical requirements include frequent standing and walking, lifting and pushing/pulling up to 50 lbs., and manual dexterity sufficient to perform all pharmacy duties
