Nacogdoches Medical Center, a community hospital with nearly 50 years of service in East Texas, is recruiting a PRN Registered Nurse Case Manager to join its care coordination team. Practice Highlights

  • Service-line–based caseload: RN Case Manager leads a defined population of patients, ensuring care is delivered at the appropriate level and in the right sequence throughout the inpatient stay
  • Utilization Management responsibilities include medical necessity review and denial prevention in alignment with state, federal, and TJC accreditation standards
  • Transition Management focus: appropriate length-of-stay management, readmission prevention, and patient satisfaction outcomes
  • Care Coordination role: drives throughput efficiency and ensures timely, safe discharge planning for patients and families
  • Compliance scope encompasses state and federal regulatory requirements and The Joint Commission (TJC) accreditation standards
  • Education responsibilities extend to physicians, patients, families, and caregivers
  • Preceptor role for new case management staff; serves as a department resource and facilitates TEMPO processes as needed
  • Active participation in department quality improvement initiatives and at least one committee or major assigned project
  • Facility service lines include: obstetrics and gynecology, cardiology (cardiac catheterization lab and electrophysiology), cancer care, neuroscience (Brainlab technology, stroke treatment program), and bariatric/weight loss surgery

Qualifications

  • Active RN license in the state of Texas (or eligibility to obtain); must meet all state law and regulatory requirements for licensure
  • Graduate of an accredited school of nursing required; bachelor's or master's degree in nursing preferred
  • Minimum 2 years of acute hospital case management experience required
  • Accredited Case Manager (ACM) certification preferred
  • Demonstrated competency in utilization management, discharge planning, and interdisciplinary care coordination
