Resolute Health Hospital in New Braunfels, Texas is recruiting an experienced Registered Nurse Case Manager to join its care management team at a full-service acute care facility offering emergency services, cardiovascular care, orthopedics, women's health, labor and delivery, and the area's only Level III NICU. This role carries meaningful clinical and operational responsibility — coordinating care across the continuum, driving appropriate utilization, and ensuring safe, timely transitions for a diverse inpatient population. - Transition Management scope includes length-of-stay optimization, discharge planning, and patient satisfaction
- Provides education to physicians, patients, families, and caregivers as a core function of the role
- Operates within national standards for care management scope of services, integrating UM, transition management, care coordination, compliance, and education
- Collaborative environment with physicians, clinical staff, and interdisciplinary teams across multiple service lines
Qualifications
- Graduate of an accredited School of Nursing required; Bachelor's or Master's degree in Nursing preferred
- Active Texas Registered Nurse (RN) license required
- Minimum of 2 years of acute hospital patient care experience required
- Prior utilization management and/or case management experience required
- Accredited Case Manager (ACM) certification preferred
- Demonstrated competency in medical necessity criteria, denial prevention, discharge planning, and regulatory compliance
