Practice Highlights

  • Full-scope inpatient pharmacy practice including order verification, medication profile review, and clinical intervention documentation
  • Clinical review responsibilities encompass duplicate therapy screening, drug–drug and drug–food interaction checks, allergy review, dosing appropriateness, and frequency verification
  • High-risk IV checking, renal dose adjustment, IV-to-PO conversions, and black box warning interventions are core daily functions
  • Collaborative practice management under facility-approved protocols: anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults
  • Medication reconciliation support and clarification of unsafe or unclear drug therapy orders
  • Direct advisory role to physicians on medication utilization and drug information
  • Supervisory oversight of pharmacy technicians and support personnel in preparation, dispensing, and maintenance processes
  • Patient education on medication use; participation in training of physicians, nurses, pharmacy technicians, and students
  • Regulatory compliance responsibilities including nursing unit inspections and adherence to Federal, State, and Joint Commission standards
  • Collaborative relationships with medical and nursing staff across the acute care campus

Qualifications

  • Pharm.D. or B.S. in Pharmacy required
  • Active Texas Pharmacy license required (or eligibility to obtain)
  • AHA BLS, ACLS, and PALS certifications required for positions with direct patient care responsibilities
  • Hospital pharmacy experience preferred
  • Board Certification in Pharmacotherapy (BCPS), Certified Geriatric Pharmacist (CGP), or equivalent board certification preferred
