About this role
Team Florida is looking for a Primary Care Physician to lead senior focused care at our South Stuart clinic on the Treasure Coast. This is a value based seat built around smaller patient panels, longer visits, and a full care team that shares your scorecard for keeping patients healthy, not a schedule built around volume. You will practice in a coastal Martin County community where seniors have chosen to retire and settle, giving you the chance to build long term relationships with the same patients year after year. With a new company forming around an established Treasure Coast practice, this is a rare moment to help shape how senior primary care is delivered in South Stuart for years to come.
What you will own:
Serve as the primary physician for a dedicated panel of senior patients, building long term relationships through more time per visit than traditional practice allows
Lead a care team that includes advanced practice clinicians, nurses, care coordinators, and behavioral health support around every patient
Develop and manage individualized care plans focused on prevention, chronic condition management, and closing gaps in care before they become problems
Review quality and outcomes data regularly, using panel level insights to guide proactive outreach to patients who need attention
Collaborate with specialists, hospital teams, and skilled nursing partners to coordinate care across every setting a patient touches
Conduct comprehensive wellness visits, annual assessments, and in office procedures appropriate to primary care
Partner with clinic leadership on referral patterns, utilization, and site level performance against shared quality goals
Mentor advanced practice clinicians and support staff to strengthen the overall care team
Engage with the South Stuart community to build trust and awareness of the practice among local seniors
What we look for:
Active and unrestricted Florida medical license as an MD or DO
Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
Active DEA registration or eligibility to obtain one before start
Experience caring for a Medicare or senior population, with comfort managing multiple chronic conditions
Familiarity with or strong interest in value based, capitated care models and quality driven outcomes
Comfort practicing collaboratively alongside advanced practice clinicians and a multidisciplinary care team
Strong communication skills and a genuine commitment to preventive, relationship based care
Willingness to work full time, in person, at the South Stuart clinic
Bilingual ability in English and Spanish is a plus
