About this role
Team Florida is seeking an Advanced Practice Clinician to join our senior focused primary care clinic in Oviedo, part of the growing Orlando market. This role operates inside a value based care model where smaller patient panels and longer visit times allow you to truly know your patients, practice proactive and preventive medicine, and partner with a full care team built around every senior you serve. You will work alongside a dedicated physician and care team with one shared goal, keeping patients healthy, and one shared scorecard built on outcomes rather than volume. This is a chance to help shape a new chapter for an established Florida practice and build a lasting career in a community known for its strong neighborhoods and quality of life.
What you will own:
Manage a dedicated panel of senior primary care patients with the time and support to deliver whole person care
Conduct comprehensive visits that emphasize prevention, chronic disease management, and early intervention
Partner daily with a physician, care coordinators, and support staff to close gaps in care before they become problems
Develop and adjust individualized care plans based on each patient's evolving health needs and goals
Engage patients and families in shared decision making and ongoing health education
Coordinate referrals and transitions of care with specialists, hospitals, and skilled nursing partners
Document care thoroughly and accurately to support quality measures and patient outcomes
Participate in regular care team huddles and quality reviews focused on the health of the panel
Support same day access and timely follow up for patients with urgent or changing needs
What we look for:
Active, unrestricted Florida license as an Advanced Practice Registered Nurse or Physician Assistant
Current national board certification appropriate to licensure, such as Family Nurse Practitioner or Physician Assistant Certified
Active DEA registration or eligibility to obtain one in Florida
Experience providing primary care to adult or senior patients, value based care experience preferred
Comfort managing chronic conditions common among older adults, such as diabetes, hypertension, and heart disease
Strong communication skills and a collaborative approach to working within a physician led care team
Commitment to preventive, relationship based care over high volume, transactional visits
Ability to work full time on site in Oviedo with reliable availability for a consistent patient panel
Bilingual ability in English and Spanish is a plus for serving the local community
