<p>A mission-driven healthcare organization in Grand Rapids, Michigan is recruiting a Licensed Master Social Worker (LMSW) to serve as Palliative Care Manager — a leadership role that combines program development, operational oversight, and direct patient care. Reporting to the CEO, you will launch and build a new palliative care program from the ground up, establishing workflows, policies, compliance frameworks, and a high-performing interdisciplinary team while maintaining hands-on social work practice as the program scales.</p>

<h3>Practice Highlights</h3> <ul> <li>Program startup leadership: develop workflows, policies, procedures, and operational processes for a new palliative care program</li> <li>Budget management and financial performance monitoring</li> <li>Supervision and support of palliative care staff in collaboration with the Medical Director</li> <li>Compliance oversight: Medicare, Medicaid, commercial payer requirements, and accreditation standards</li> <li>Provider credentialing and payer contracting support</li> <li>Operational management: scheduling, referrals, clinical documentation, and care transitions</li> <li>Direct social work service delivery during program launch and as needed</li> <li>Participation in hospice Manager On-Call rotation</li> <li>Focus on program growth, customer service, quality improvement, and operational efficiency</li> </ul>

<h3>Qualifications</h3> <ul> <li>Master's degree in Social Work required</li> <li>Active LMSW credential required</li> <li>Hospice or palliative care leadership experience preferred</li> <li>Program development or startup experience preferred</li> <li>Proficiency with electronic medical records (EMR) and Microsoft Office applications</li> <li>Strong leadership, communication, interpersonal, and organizational skills with ability to lead through growth and change</li> </ul>
