Post-Acute Care NP/PA

StaffMed Health Partners is recruiting a Nurse Practitioner or Physician Assistant to join a Post-Acute Care team serving skilled nursing and rehabilitation facilities in Cortland, NY. ### What you'll do - Provide clinical care to patients in SNF and rehabilitation settings five days per week, Monday through Friday - Manage post-acute patient panels with support from a collaborative interdisciplinary team - Document and bill under a fee-for-service productivity model - Utilize EMR systems with dedicated training and ongoing support provided - Partner with back-office and administrative staff to maintain efficient patient throughput ### What we're looking for - Current NP or PA-C certification - Active New York state license or demonstrated ability to obtain one - Comfort working independently in a post-acute or SNF environment (prior experience preferred, not required) - Strong clinical judgment in managing complex, medically fragile patient populations ### What's included - Base salary range of $130,760–$154,911 annually - Fee-for-service model with uncapped productivity bonuses - Full benefits package including health, dental, and vision coverage - Paid professional liability insurance with tail coverage - CME allowance and leadership development opportunities - EMR training and back-office administrative support ### About the practice StaffMed Health Partners is a national healthcare staffing and practice management organization placing advanced practice providers in post-acute care settings across the country.
