Post-Acute Care Physician

StaffMed Health Partners is recruiting a Post-Acute Care Physician to join a national healthcare organization's SNF/rehabilitation team in Syracuse, NY. ### What you'll do - Provide medical care to patients in skilled nursing and rehabilitation facilities across the Syracuse area - Manage a patient panel on a 4-day-per-week schedule with day-call weekend rotation and after-hours backup coverage - Collaborate with an interdisciplinary post-acute care team to coordinate patient treatment plans - Document clinical encounters using a supported EMR platform with dedicated training resources - Leverage back-office and administrative support to maintain efficient daily workflow ### What we're looking for - MD or DO degree required - Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics - Active New York medical license or ability to obtain - Post-acute care (SNF/rehabilitation) experience preferred; new entrants to the setting considered ### What's included - Base salary range: $190,046–$225,055 annually - Fee-for-service model with uncapped productivity bonuses - Full benefits package including health, dental, and vision - Paid professional liability insurance with tail coverage - CME allowance and leadership development opportunities - EMR training and ongoing technical support ### About the practice This role is part of a national post-acute care organization with an established presence in Central New York, providing physicians with back-office infrastructure and a collaborative clinical environment.
