About this role
AdventHealth Celebration is hiring a full-time Heart Failure Clinical Coordinator to lead evidence-based care coordination and program management for heart failure patients at their Celebration, Florida campus.
### What you'll do
- Coordinate a multidisciplinary approach to ensure patients receive timely, appropriate medical and supportive services across the care continuum
- Develop and implement patient education programs grounded in evidence-based standards
- Monitor and evaluate program data to establish, track, and achieve clinical and operational goals
- Conduct data analysis to support pay-for-performance reimbursement efforts
- Facilitate problem resolution across departmental, campus, and physician/provider lines
- Participate in community outreach through physician visits, educational seminars, and community lectures
### What we're looking for
- Active Registered Nurse (RN) license in Florida (required)
- Minimum 3 years of nursing experience (required)
- Diploma in Nursing (required); Bachelor's or Master's in Nursing (preferred)
- Strong team-centered leadership, facilitation, and process improvement skills
- Excellent organizational and communication skills with ability to engage diverse populations
- Critical thinking and problem-solving ability with capacity to manage competing priorities independently
