About this role
UChicago Medicine AdventHealth is hiring a full-time RN Case Manager at its Hinsdale, Illinois hospital campus, working Monday–Friday day shifts with occasional weekend and holiday rotation.
### What you'll do
- Participate in multidisciplinary rounds to review patient status, progression, level of care, and discharge planning for all assigned patients.
- Communicate with and educate patients and families on the emotional, social, and financial impacts of illness while mobilizing community resources.
- Assess readmitted patients and families to identify perceived drivers of readmission and coordinate appropriate follow-up.
- Organize and facilitate patient and family care conferences with the multidisciplinary team.
- Document discharge planning evaluations, ongoing assessments, barriers to progression of care, avoidable days, and patient and family needs per standard work protocols.
### What we're looking for
- Active RN license (required); ADN required, BSN preferred.
- Minimum two years of medical/surgical or hospital nursing experience.
- Prior Care Management or Utilization Management experience preferred.
- CCM or ACM certification preferred.
- Strong critical thinking, prioritization, and multitasking abilities with demonstrated customer service skills.
