About this role

AdventHealth Lake Nona is hiring a Director of Care Management Social Worker to lead its care management department at its Orlando, Florida campus, a 60-acre health and wellness facility opening in late 2026. ### What you'll do - Direct daily operations of the care management department, including discharge planning, care coordination, progression of care, and social services - Oversee recruitment, retention, and leadership development to maintain adequate staffing within care management - Partner with Utilization Management for the Utilization Review Committee and day-to-day operations, ensuring compliance with CMS Conditions of Participation for Discharge Planning - Engage post-acute care entities to improve patient care transitions and reduce readmissions - Develop and manage the department's operational budget, ensuring capital and fiscal accountability - Identify performance improvement opportunities and implement, monitor, and report on action plans using data and outcome metrics ### What we're looking for - Master's degree in Social Work, or a Bachelor's in Social Work combined with a Master's in a related field (required) - 5+ years of direct care management experience (required) - 3+ years of care management leadership experience (required) - 2+ years of social work experience (required) - Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification (preferred) - Strong working knowledge of CMS Conditions of Participation for Discharge Planning, managed care, and value-based programs ### What's included - Salary range: $91,314 – $169,846 annually - Medical, dental, vision, life, and disability insurance from day one - Paid time off from day one - 403(b) retirement plan - 4 weeks of 100% paid parental leave - Career development and whole-person well-being resources ### About the practice AdventHealth Lake Nona is part of AdventHealth, a faith-based, not-for-profit health system serving communities across the United States.
