Senior Manager Care Management RN

AdventHealth is hiring a Senior Manager of Care Management RN for its inpatient, observation, and emergency department operations at its Altamonte Springs, Florida facility. ### What you'll do - Manage daily care management operations across inpatient, observation, and ED settings, driving strategies to reduce length of stay and avoidable readmissions. - Lead and participate in daily multidisciplinary rounds to address discharge barriers and facilitate timely patient transitions to post-acute settings. - Oversee staff utilization of technology and adherence to standard work for discharge planning in collaboration with patients, families, care teams, and payors. - Monitor dashboards and performance data to ensure staff compliance with documentation and regulatory requirements. - Coach and educate care management staff on accurate documentation of patient evaluations, discharge plans, and CMS requirements. - Participate in hiring, onboarding, and retention of department staff while leading regular staff meetings and process improvement initiatives. ### What we're looking for - Active Registered Nurse (RN) license required. - Bachelor's degree in Nursing required; Master's in a related field preferred. - 4+ years of care management experience required; 2+ years of frontline nursing experience required. - Solid working knowledge of CMS Conditions of Participation, medical necessity criteria, and managed care/value-based programs. - ACM or CCM certification preferred. - Nursing leadership experience and inpatient care management background preferred. ### What's included - Compensation range: $85,530–$159,090 annually. - Medical, dental, vision, life, and disability insurance effective day one. - Paid time off from day one and 4 weeks of fully paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice AdventHealth is a faith-based, not-for-profit health system with a broad hospital and clinic network across the United States.
